This form gives Triple Win Waco the permission to use photos/videos of you (or the minor you sign for), for promotional materials. This form's return, with either permission or denial of permission, is required for students to participate in Triple Win Waco Summer Camps.

As the legal guardian of the camper identified above, I give my permission to Triple Win Waco to use my child's photograph/video as needed for advertisements, publications, videos, web pages, social media, campus art and other informational pieces. By selecting the “I give my permission” button, I am signing this document electronically. I agree that my electronic signature is the legal equivalent of my manual/handwritten signature on this document. I further agree that my signature on this document is as valid as if I signed the document in writing. I am also confirming that I am authorized to enter into this Agreement. If I am signing this document on behalf of a minor, I represent and warrant that I am the minor’s parent or legal guardian.
